What is the Crisis Resolution Team?  

The Crisis Resolution Team (CRT) supports families with youth and young adults who are experiencing behavioral health challenges and would benefit from intensive, short-term, in-home services and linkage to ongoing support.

Services can include needs assessment, service planning, care coordination, crisis management, peer support, family skill building, individual/family therapy, psychiatry, and medication management.

CRT aims to prevent unnecessary hospitalization or other out-of-home placement whenever possible by wrapping services around the family in an effective manner. 

Meet the Team

    Erik Cooper, LMFT

    CRT CLINICAL SUPERVISOR AND THERAPIST FOR MONTROSE & DELTA COUNTIES

    Erik (he/him) is a marriage and family therapist with over 30 years of experience and is the author of “Shall We Dance: A Guide to Happiness.” Erik works with clients with a wide range of concerns including: depression, marital problems, anxiety, individual psychological problems, and child-parent problems.

    Erik is also trained in EMDR and TFCBT (Trauma Focused Cognitive Behavioral Therapy) – helping clients who have experienced trauma, physically, emotionally, mentally and or spiritually.

    Erik believes of the most important building blocks for growth in therapy is the relationship between the individual and the therapist. Because of this, Erik’s approach is different with each individual, depending on their specific needs.

    He can help you regain your perspective and focus by providing you with tools to help you get back on track. It’s time to exchange fear, anxiety or panic for peace.

    Laurie Devere

    ADMINISTRATIVE COORDINATOR

    Laurie (she/her) is a Colorado native who believes in the power of connection, compassion, and community. As an administrative professional, she brings heart and intention to everything she does—whether it’s supporting a team, coordinating the moving pieces behind the scenes, or making sure the people around her feel seen and supported.

    Rooted in a deep passion for whole health, social justice, and equitable systems, she approaches her work with a lens of care and curiosity. Her lived experience as a mom of two and a dog lover adds perspective to her ability to navigate complexity with warmth and humor.

    Driven by a belief that small acts of organization and kindness can lead to big change, she shows up each day committed to fostering spaces where people—and systems—can thrive. When she’s not at work, you’ll find her immersed within her community,  finding joy in the everyday, and staying grounded in the values that guide her.

    Yesenia Duncan

    CRT PROGRAM DIRECTOR

    Yesenia (she/her) obtained a Bachelor’s Degree in Human Communication and a Bachelor’s Degree in Journalism with an emphasis in Telecommunications. She has over 15 years experience in working with youth and families in conflict along with those involved in the judicial system.

    Her work experience includes working in residential facilities for youth as well as a Probation Officer working with individuals struggling with finding their way. Her knowledge of systems including the Child Welfare System and other community agencies is a benefit to assisting others navigate challenging processes. Her personal journey as a mom and wife add to her repertoire of knowledge.

    Her passion to begin working with others as a Coach was fueled by what she loved the most in her previous careers: celebrating small wins and seeing those that never believed in themselves get a spark back.

    Her ultimate goal is to help youth, young adults, parents and families work through their own fears and judgments to help them become the best version of themselves and increase individual and family resilience.

    Erin Fenn, MS, CAT

    THERAPIST FOR MESA COUNTY

    Erin (she/her) obtained a Bachelor's degree in Healthcare Administration and completed her master’s degree in Licensed Addiction Counseling. Erin’s work experience includes working in residential treatment and detox facilities.

    Erin is excited to share her own experience, strength, and hope, while supporting healing and recovery within the lives of others. Erin enjoys being a part of the CRT team, and believes that the essential elements of care include understanding individual needs and collaboration between community partners.

    In her spare time, Erin enjoys spending time at the lake, bbq’s, and time with the family. She feels blessed to live in the beautiful state of Colorado, and lives for the mountain sunsets. Erin is always up for watching sports, and believes that great conversation is food for the soul!

    Alyssa Hollos

    PEER SUPPORT FOR MESA COUNTY

    Alyssa (she/her) serves as a Peer Support Specialist for Mesa County. She holds a Bachelor’s degree in Sociology with a minor in Psychology from Colorado Mesa University.

    Alyssa brings powerful lived experience to her role, offering empathy, connection, and hope to those navigating challenging seasons. She is passionate about making the world a better place, one relationship at a time, and believes that healing is possible through shared understanding and authentic support.

    In her spare time, Alyssa enjoys kayaking and spending time with her beloved pets—her dog Athena and her cats Totoro, Chihiro, and Pon Kichi. She finds joy in the calm of nature and believes that even small moments of connection can make a big difference.

    Allison Luna

    PEER SUPPORT FOR MONTROSE & DELTA COUNTIES

    Before starting her journey as the Peer Support Specialist with the CRT team, Allison (she/her) worked in the healthcare field as a CNA for 7 years and a Registered Medical Assistant for a year. After a recent life changing event, she decided she wanted to continue to help people in a different way. With empathy being one of the most important aspects of life to her, she wanted to join a team that not only helps families and individuals, but she wanted to do so in a way that her own life experiences - good and troubling - could relate to those she gets to meet, so she can practice and share empathy and learned life skills with whomever she can.

    When Allison is not at work, she is spending time with her husband, 7 children, 2 dogs, and 2 cats. Some of their favorite things to do together are fishing, hiking, swimming, camping, spiritual activities, walking, singing Disney songs, and playing games together.

    Allison and her family moved to Colorado two years ago from Washington State where they lived for over 20 years. Before that, Allison lived in Canada - where she was born - for 14 years. Colorado has quickly become home to her and her family and she is excited to meet new people, and explore as much as possible.

    Nichole Ovington

    CASE MANAGER FOR MESA, MONTROSE & DELTA COUNTIES

    Nichole (she/her) obtained a Bachelor's of Social Work with a minor in Youth Ministry and moved to Colorado shortly after. She has worked with foster parents and foster youth for over 8 years.

    She believes in meeting people where they are at with encouragement and support; working to come alongside them in partnership. Utilizing local resources and creative problem solving she aims to help equip families with the tools they need to thrive.

    In her spare time, you may find Nichole hidden in the pages of a book, working on puzzles, or venturing to state festivals or events.

  • Nikole Stanfield, MA, LPCC, NCC

    THERAPIST FOR MONTROSE & DELTA COUNTIES

    Sometimes life can be messy. As someone who has personally experienced burnout, anxiety, and struggling to identify a career, Nikole knows what it is like to feel stuck. Raising your hand and saying, “I want something different,” can be hard and make you feel vulnerable.

    Nikole (she/her) aims to collaborate with you as you take steps toward the change you want in your life. You don’t have to be on this path alone. Together, we can identify the change you are looking for and collaborate to work towards your goals. At the same time, Nikole understands that there is no right way to make a change. Taking the first step is the hardest part.

    As a native Coloradoan, Nikole loves getting out on the water on her stand-up paddle board, taking her dogs for walks, exploring the local mountains, and enjoying all the fresh produce that grows on the Western Slope. Nikole has had several businesses since 2014, and recently completed her master’s degree in clinical mental health counseling from Adams State University.

    Roger Trembly, MSW, SWC

    THERAPIST FOR MONTEZUMA, LA PLATA & ARCHULETA COUNTIES

    Roger (he/him) grew up in the southwest Colorado mountains and loves everything the San Juans have to offer. He received his MSW from the University of Denver Four Corners MSW Program with a focus on rural social work and received his BA in psychology from Fort Lewis College. Roger has over 10 years of experience working with children and families through social services and child welfare programs.

    Roger's preferred method is a person-centered and strengths-based approach aligned with a good therapeutic relationship. Roger enjoys working to not only identify individual strengths and supports but also build upon them. He realizes participating in therapy is a daring and intimidating venture and offers support and compassion to help meet the client’s needs. Roger is passionate about helping individuals explore and grow into the best version of themselves.

    In his spare time, Roger enjoys mountain biking, spending time outside, camping in the mountains, and connecting to nature. Roger can also be found spending time with family and friends around the BBQ grill cooking a feast.

    Isabelle Vaughan

    CASE MANAGER FOR MONTEZUMA, LA PLATA & ARCHULETA COUNTIES

    Isabelle (she/her) obtained a Bachelor's in Sociology and Human Services from Fort Lewis College in 2021.  She spent the past 3 years working at a family center where she specialized in case management and resource navigation.  Her experience includes serving families who screened out of Child Welfare and working with fathers in a curriculum-based group to gain tools and support in their fathering journey.

    Isabelle was inspired by her own life experiences to pursue a career in case management and help children who have had adverse childhood experiences. She believes in working with families in a strengths-based manner and meeting them where they are at. 

    When she's not working, you can find Isabelle spending time with her husband and children enjoying meals together and playing board games. She also loves reading, cooking, and checking out local coffeeshops.

    Hope Woods

    PEER SUPPORT FOR MONTEZUMA, LA PLATA & ARCHULETA COUNTIES

    Before starting her journey as peer support for the Crisis Resolution Team, Hope (she/her) was a health and nutrition coach. She delighted in folks achieving what was most important to them, just by having someone to keep them accountable to themselves and their own goals. Like them, she was working to achieve her own goals, and trying to heal from the childhood trauma that was keeping her stuck in certain ways.

    During this time, she received a lot of support from my community in her healing--from therapists, self-help books, famous inspirational speakers, and support groups, and at some point decided she wanted to be able to give that back in some way. It then became apparent to me that she really wanted to work with youth and families.

    In her free time, she loves reading and spending time in nature--hiking and adventuring with her partner and son. They love exploring the mountains of southwest Colorado and hiking the deserts of Utah and Arizona.

    Wren Zeyrek

    CASE MANAGER FOR MESA COUNTY

    Wren (she/her) earned a Bachelor’s degree in Human Services from Metropolitan State University of Denver.

    She is passionate about supporting families on their journey toward stability by helping them identify their strengths and reach their goals. Wren believes that every family is unique in its culture, structure, and story, and is dedicated to creating safe, welcoming spaces where all families feel supported.

    In her spare time, she enjoys fussing over her dog and houseplants, escaping into books, creating art, and spending time in nature.

Community-Based Wraparound Services for Families in Crisis Work Plan

Program Goals and Objectives 

The purpose of the Community Based Wraparound Services for Families in Crisis (CWA)  is to provide short-term in-home services to children, youth, and families in a behavioral health crisis.

Evidence demonstrates that young people and families experience improved outcomes when stabilized in the home which reduces the burden on the bed-based system.

This pilot aligns with the goals of the Behavioral Health Administration (BHA) in several important ways: CWA enhances the range of behavioral health services available to children, youth, and families in rural Colorado; it meets families where they are; and it supports Colorado’s vision to make the right level of care at the right time accessible to all. It also requires that partners in behavioral health collaborate effectively to achieve program effectiveness. 

Program Scope and Geography 

Rocky Mountain Health Plans is contracting with Collaborative Trauma Solutions (CTS) to have three CWA teams serving the following areas: 

  • Montrose/Delta

  • Mesa

  • Montezuma

  • La Plata/Archuleta
